Survey: Healthy Food and Beverage Sales

The Creston & District Community Complex is evaluating the benefits of providing Healthy Food and Beverages as part of the food services aspect of the Community Complex. It is important for the Community members who plan to use this facility to give your feedback for the planning process.
Thank you for taking the time to answer a few questions to assist with this planning.

1.  On a scale of 1 to 5, how would you rate the importance you give to healthy eating?

2.  Would you like to see more options for healthy choices available?
Yes, I would visit concession stands more often if healthier choices were available.
No, I would not visit concession stands more often.

3.  Would you buy healthy food at the concession/juice bar if it was offered at: (check all that apply)

4.  Would identification of healthier items be helpful? (example - a sticker)
Yes, I would definitely buy them more often.
No, identification (stickers) would not make a difference.
